Cardiff & Vale University Health Board Share ‘Leave Nobody Behind’ Form For Priority Groups 1-9

 

Cardiff and the Vale University Health Board have taken to social media to share the ‘Leave Nobody Behind’ form for those in Priority Groups 1-9 who have yet to be offered the Covid-19 vaccine.

Priority Groups 1-9 cover the first phase of vaccinations here in Wales and include the following:

  • Residents in a care home for older adults and their carers
  • All those 80 years of age and over and frontline health and social care workers
  • All those 75 years of age and over
  • All those 70 years of age and over and clinically extremely vulnerable individuals
  • All those 65 years of age and over
  • All individuals aged 16 years to 64 years with underlying health conditions which put them at higher risk of serious disease and mortality
  • All those 60 years of age and over
  • All those 55 years of age and over
  • All those 50 years of age and over

It has been estimated that, together, these groups represent around 99% of preventable deaths from Covid-19.

The purpose of the ‘Leave Nobody Behind’ form is to ensure that all people in those groups, and therefore eligible for the vaccination, are offered the opportunity to receive the jab.

In their statement on social media, Cardiff and the Vale University Health Board said:

“If you are in priority groups 1-9 and have not yet received your COVID-19 vaccine, please complete our ‘Leave Nobody Behind’ form. Fill in your details and we will be in touch to arrange your vaccination appointment.”

More than a fifth of people in Wales have now received both their first and second dose of the vaccine, with a total of over 1,727,455 people in Wales having been given their first dose.
